Emergency Treatment Authorization for Minors
In the event you are not with your child when an emergency situation occurs, you should know that your child cannot receive treatment without your consent. When leaving your child in the care of others, this signed release form will help your child avoid unnecessary delay of treatment. (If a physician feels immediate care is necessary to prevent death or serious injury, treatment will begin.) To ensure immediate medical attention for your child in your absence, complete this information and leave it with your child’s caregiver. Download the form here.

For MercyOne clinic patients, MercyOne MyCare is an online tool that gives you direct access to your personal health information. Your lab results, appointment information, medications, health history, and more are all available in a secure online environment. With MyHospitalCare, you can have direct access to your:
- Lab results
- Medications
- Discharge instructions for what you need to do after your hospital stay
- Health history including immunizations
- Allergy list
Make sure you give your e-mail address at check-in or to a member of your care team. You will receive an e-mail from Cerner Health after your hospital stay, so please watch for that e-mail.

Please note that if you are a patient of a MercyOne clinic, you may also have a separate MyCare account for your health information from your physician office. MyHospitalCare gives you information from your hospital stay only.
