Population Health Care Management Program (PHCMP)


MercyOne’s nationally acclaimed Care Management program is a core component of MercyOne’s PHSO model. With a continuum of care approach our team of over 200 Care Management team members ensure seamless, coordinated care across the continuum to improve quality, increase satisfaction and contain costs. 

All MercyOne Clinically Integrated Networks (Chapters or Markets) have a single reporting structure for Care Management that expands the continuum. In the Ambulatory environment this means a cross functional team of Community Health Workers, Health Coach Assistants and, Health Coaches. While connecting the complex environment of Acute and Post-Acute settings, MercyOne brings together a robust embedded team of Social Workers, Utilization Management, Acute Case Management and Post-Acute Care Coordinators.

As the pioneer in Health Coaching, all Care Management staff are trained on MercyOne’s proprietary Health Coach training that brings together Social Influencers of Health, Shared Decision Making and Motivational Interviewing. Helping patients navigate the complexity of healthcare and chart their path to improved health.

What is a MercyOne Health Coach?

MercyOne Health Coaches are licensed, registered nurses who work with primary care doctors to provide one-on-one care to help patients set and achieve health improvement goals.

 

Who do MercyOne Health Coaches Help?

MercyOne Health Coaches are available to work with patients with chronic conditions such as diabetes, heart disease and high blood pressure to provide education and support during the journey to a healthier lifestyle. MercyOne Health Coaches also remind patients when it is time for their regular health screenings and wellness exams.

 

How does the MercyOne Health Coach program work?

MercyOne Health Coaches work with patients to create a customized health improvement plan to meet each individual’s specific needs. The plan may include education about specific conditions or risk factors, healthy eating tips, an exercise plan or a timeline for follow-up visits. Most importantly, MercyOne Health Coaches provide ongoing support to patients as they make changes to improve their health.

 

Where can I find a MercyOne Health Coach?

MercyOne Health Coaches are stationed at family practice, internal medicine and pediatric sites throughout MercyOne and are available to help all patients. To schedule an appointment with a MercyOne Health Coach, simply call your regular doctor’s office and ask to speak with the Health Coach.

 

What is the cost of MercyOne Health Coach services?

Services provided by MercyOne Health Coaches are free to patients who visit a primary care doctor throughout MercyOne.
