Your employer has chosen MercyOne Occupational Health for health services related to your employment, to ensure a safe and healthy work environment and to comply with federal or state regulations. Please read the following to make your clinic visit as efficient as possible:
- Arrive on time or a few minutes before your appointment time and proceed to the front desk. You may be asked to complete some forms at that time.
- Always bring a photo I.D. with you to all clinic visits.
- If you normally wear glasses, contacts, or hearing aids, please wear them or bring them with you to your appointment.
- If you are a minor (under age 18), your parent or guardian must complete the Consent Form. Please bring the completed form with you to your appointment. If your parent/guardian accompanies you to the appointment, they can complete the Consent Form at that time.
- For specific instructions (e.g., drug testing, DOT physicals, mask/respirator fit testing, etc.), please refer to the Resources section.
