Your employer has chosen MercyOne Occupational Health for health services related to your employment, to ensure a safe and healthy work environment and to comply with federal or state regulations. Please read the following to make your clinic visit as efficient as possible:
- Arrive on time or a few minutes before your appointment time and proceed to the front desk. You may be asked to complete some forms at that time.
- Always bring a photo I.D. with you to all clinic visits.
- If you normally wear glasses, contacts, or hearing aids, please wear them or bring them with you to your appointment.
- If you are a minor (under age 18), your parent or guardian must complete the Consent Form (insert link to pdf here). Please bring the completed form with you to your appointment. If your parent/guardian accompanies you to the appointment, they can complete the Consent Form at that time.
- For specific instructions (e.g., drug testing, DOT physicals, mask/respirator fit testing, etc.), please refer to the Resources section.
