Documentation and what to bring

Please make sure to complete any requested forms in advance when possible. It’s also recommended to bring the following to your appointment:

• A photo ID and current insurance card
• A list of medications, vitamins and supplements
• Relevant health records or updates to your medical and family history
• A list of symptoms, questions and goals you want to discuss
